The captive's limbs are inside of the animatronic's. The interior is quite cavernous, large enough to hold an adult inside. He also has numerous boils on the right side of his head, along with his ears and left eye socket. With the illusionary disc on, He transform into a larger version of the original Freddy Fazbear, he has a gaping opening in his belly, with numerous sets of teeth inside, similar to the teeth on the stomachs of Nightmare Fredbear and Nightmare. His head appears to be squarish and less bulky than Withered Freddy from Five Nights at Freddy's 2, but not as round as one in Five Nights at Freddy's.
In his normal form, Twisted Freddy resembles a silver version of his funtime counterpart from FNaF: Sister Location, only recognizable as Freddy by the ears and hat.
